How to fill out the T2201 Disability Tax Credit Form?

The form consists of two parts, both of which you have to complete. Once you have filled out both sections, you can submit the form to the CRA. Before you start the process, it is best to do a free self-assessment form that you can find here.

As for the two parts of the T2201 Disability Tax Credit Form, you will have to fill out your personal information and describe your disability in the first part, while the second part is where a health professional explains the details and nature of your disability. Both parts have to be filled out in full.

Part A –  is where all your basic personal information goes; such as, your name, address, SIN number and date of birth. If you are completing the form on behalf of the disabled individual, there is a section specifically made for you.

Part B – This section is for the medical practitioner to fill out. It contains sections about each impairment that restricts your life.
